Please look over the Night Nanny Employment Checklist below and complete all the steps.

Present current documentation of Infant & Child CPR certification (must send documentation).

Experience working with infants for at least two (2) years.

Furnish proof of current TB test - to be updated every 2 years (must send documentation).

Complete Pertussis/Tetanus Booster for Adults (must send documentation).

Supply an approved infant care training form or approved waiver (must have a letter of documentation from trainer).

Deliver approved hands on multiple's training documentation or approved waiver (must have a letter of documentation from trainer).

Complete the Night Nanny Application.

Review and sign the Night Nanny Contract.

Review Liability Insurance options.

Provide a copy of your resume.

Complete and signed Background Check Release Form.

Fill out and return W-9 Form (include copy of Social Security Card and Driver's License).
